Application for Reinstatement to SSS
Please use the following form if you received a notice from SSS that you are no longer an "active" student and would like to be considered for reinstatement. Once you complete the form, an SSS staff member will be in contact with you to schedule a reinstatement meeting.
IMPORTANT: You only have 30 days from notice of status change to complete the application for reinstatement. If you fail to complete the application within the deadline, you will not have further opportunities to do so.
